Our Reception Staff is available to assist you with registration, appointments, questions, and disability form completion. When your call comes through our mainline – (916) 454-6677 – they will make every attempt to help you and/or direct you to the appropriate person or department.
Registration
Upon arrival, we will ask you for any paperwork you may have received from our office, your insurance information, and any previous records you were asked to bring when you made your appointment. Also, we will need a PHOTO ID (a Federal Law Requirement). You will be asked to sign an acknowledgement of access to our HIPPA Privacy Practices – posted in our office and also available on the link below. Your co-pay will be collected when you check in. For your convenience, we take Visa and Mastercard.

Appointments
Whether you are a new patient to our practice, or returning for a follow-up visit, please arrive on time. If you need to cancel your appointment please give us as much notice as possible so that we might open our schedule for another patient. If you need to re-schedule your appointment, we will make every effort to accommodate your scheduling needs.
Disability Form Completion
Please submit your form to our receptionists. We will keep a record of the date received, and the date we mailed them for you. A copy will also be kept in your medical record. We make every effort to complete these forms as quickly as possible; generally, please expect a 3 to 5 day processing period.
